As the parent/guardian of the above player, I certify that he/she is in excellent health and has no physical, mental or emotional problems that may be likely to prevent participation in strenuous physical play at soccer camp, clinic or matches. I agree to hold harmless Tacoma Elite Soccer Academy LLC and it's agents and employees and hereby release them from any liability on account of injury sustained during such participation and certify that he/she is covered by medical insurance which will reimburse Tacoma Elite Soccer Academy LLC for expenses incurred by them, their agents and employees on account of medical procedures ordered at their discretion and also indemnify them from any expenses not reimbursed by such insurance. I give consent for my child/player to be photographed, videotaped or filmed while participating in the clinic/camp/game and or resulting images to be used by Tacoma Elite Soccer Academy LLC for educational and promotional purposes.

I am aware there are risks to me and my child of exposure to directly or indirectly arising out of, contributed to, by, or resulting from an outbreak of any and all communicable disease, including but not limited to, the virus “severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2)”, which is responsible for Coronavirus Disease (COVID-19) and/or any mutation or variation thereof.

I understand that make-up sessions are at the discretion of the head coach in consult with the academy director/Pierce County, to schedule any extra sessions making up for time lost due to cancellation. It is at the discretion of the academy director to reschedule canceled sessions. However, due to many circumstances, make-up sessions are not always possible and, therefore we will only guarantee one make-up session.
